How to Enrol

How do I enrol at Tōtara Health?

Tōtara Health is currently accepting new enrolments at our Flaxmere clinic.  

Our Nelson Street clinic is not currently accepting new patient enrolments. Our Flaxmere clinic is accepting new enrolments for patients living in the Flaxmere area only. Please phone Totara Health on 0800 222 757 to enquire about enrolling or visit our clinic to get an enrolment form. All patients aged 16 years and older must sign and date their own enrolment form.

What do I need to bring with me to enrol?

If you were born outside of NZ:  Tōtara Health can only enrol you if you have a work visa or permit that lets you live and work in New Zealand for at least 2 years, or if you are a NZ resident or Citizen.  Please bring your passport and visa with you when you enrol. 

If you were born in NZ: Please bring a copy of your birth certificate or passport.  If you are unable to bring this with you, please talk to our receptionist as we may still be able to enrol you.

Can I see a Doctor straight away when I enrol?

If you need to see a doctor before we have confirmed your enrolment an appointment may be made in our urgent clinic for urgent medical needs only.  Please phone to make an appointment on 0800 222 757.  

If you have been enrolled with another Doctor, we will request your notes from them.  Once your notes have arrived we will contact you to welcome you to the practice and confirm your enrolment with Tōtara Health.  You can then make an appointment to see your regular doctor or nurse.
